(a) For two equal positive charges: field lines emanate from each charge and repel each other, with a null point (E=0) exactly at the midpoint of the line joining them. Equipotential surfaces are small spheres around each charge individually (close to each charge), merging into larger dumbbell/peanut-shaped closed surfaces further away — always perpendicular to the field lines.

(b) Even though every point on a given equipotential surface has the same potential $V$, the magnitude of $E$ can differ from point to point on that surface. This is seen from the drawing: where field lines are closer together (denser) on the equipotential surface, $|E|$ is larger; where they are more spread out, $|E|$ is smaller. So $E$ need not be constant in magnitude over an equipotential surface, even though it is always perpendicular to it.
